Before you apply, we ask that you complete this form so we can help find the right fit for your home and lifestyle. Please read each statement below and check the box to confirm you understand.

I understand that once I adopt a pet, any future medical care, housing costs, or costs incurred due to the pet are my responsibility.

I understand that all adopters must be at least 18 years of age to adopt a cat or dog.

I understand that if I experience a life-changing event that prevents me from keeping my pet, I will make every effort to find a loving home for them. If a proper home cannot be found, I can always return my pet to NEAHS.

Before You Bring Them Home

Adopting from NEAHS includes a full Adoption Agreement, which you'll review and sign in person at pickup. It covers care requirements, spay/neuter and vaccination follow-up, and our return policy. We'll walk through it together, so you'll have a chance to ask questions before you sign.
